Job Description
Job Summary: The Clinical Intake Coordinator performs a variety of duties both clinical and clerical in nature. This position requires skill in planning, organizing, and coordinating the delivery of patient care at the point of the patient’s referral. The Clinical Intake Coordinator has direct contact with referring physicians, hospital liaisons, and patient/families as it relates to the delivery of patient care at the point of a patient’s referral.
Essential Functions
Gathers pertinent information about the referred patient, including demographic, clinical, and payer source information, and completes a written referral form
Obtains the official physician’s order for start of care
Provides the Billing Department with complete and accurate insurance information
Verifies current primary physician and/or physician responsible for signing the Plan of Care
Assures appointments are made for patient follow-up after discharge from facilities
Faxes and tracks Continuance of Care Orders
Obtains face-to-face documentation from the physician’s office, reviews for appropriate content to support Medicare/Medicaid and State Regulations, reports on a weekly basis on outlying documents
Records in the EMR accepted and all declined referrals with reason for decline
Daily follow-up with Agency patients that are transferred to the hospital or other facilities
Receives resumption visit notes and tracks and obtains signed resumption orders
Conducts and coordinates morning meetings with office staff for admissions, discharges, declines, resumptions and pending referrals
Attends weekly meetings for outstanding orders, F2F, Plan of Care
