We are seeking a full-time Community Resource Coordinator for an on-site position in Colorado Springs, CO. This role involves coordinating community outreach initiatives, managing cases, and fostering relationships with community partners to connect individuals with essential services. The Coordinator will serve as a key liaison, helping patients and their families navigate available social services and resources tailored to meet their needs. Additionally, the role requires conducting training sessions, communicating effectively with stakeholders, and providing support to maintain the highest standard of service.
- Strong Communication skills to interact with patients, families, and community partners effectively.
- Experience in Case Management and Social Services to ensure the efficient delivery of care and resource coordination.
- Community Outreach expertise to establish and maintain collaborative relationships with local organizations.
- Ability to conduct Training sessions to educate staff or stakeholders as needed.
- Passion for assisting individuals and communities by connecting them with appropriate resources and services.
- Bachelor’s degree in Social Work, Human Services, or a related field preferred.
- Familiarity with local community resources and programs in Colorado is a plus.
