SUMMARY:
Provide jail based discharge liaison services to enable identified individuals to make informed choices about opportunities and services, assuring timely access to needed assistance, providing opportunities and coordinating all services to meet each consumer’s goals and thereby assisting them in achieving their maximum level of independent functioning.
EDUCATION AND EXPERIENCE:
Education: Bachelor’s degree in behavioral health related area or administration of justice required.
Licensure: Must possess or obtain highest level of licensure appropriate to degree. Valid driver’s license.
Certification: Must possess or obtain highest level of certification appropriate to degree.
Experience: Experience in mental health, substance abuse or intellectual disabilities preferred.
Knowledge/Skills: Verbal/written communication skills.
Ability to maintain composure in difficult situations.
Ability to safely operate motor vehicle.
EQUIPMENT:
Computer, fax, copier, motor vehicle and any other equipment required to perform the functions of the position.
MAJOR DUTIES AND RESPONSIBILITIES:
1. Primary liaison between Jail Authority locations, Community Services Boards (CSB), and community services and resources regarding consumer discharge needs.
2. Conduct interviews with individuals and/or family members in a therapeutic manner so as to obtain critical and thorough information, provide written documentation of information obtained and exercise good judgment in evaluating situation, making decisions and implementing effective strategies.
3. Complete initial service recommendations for consumers screened with serious mental illness during incarceration and facilitate acquisition of outside records and risk assessments to assist with establishing stability while in
4. Provide written documentation to complete all necessary chart work in accordance with policy on admissions treatment and discharges in a timely manner.
5. Maintain client confidentiality according to Frontier Health policies.
6. Remain current in knowledge of community resources and how to access those resources.
7. Keep supervisors apprised of all critical or emergency situations.
8. Coordinate discharge planning between Jail Authority and Community Service Board providers, including hand-off to the CSB of residence upon release from the Jail Authority facility.
9. Provide outreach, coordinating and monitoring services during the first thirty (30) days following discharge from the Jail Authority in order to ensure the consumer’s follow-up arrangements and needs are met.
10. Participate in in-service training and orientation and provide consultation and education services to the community as requested by supervisor/administration.
11. Participate in meeting, at Jail Authority locations, CSB offices and other regional location to ensure operations are consistent with regional standards and changes are made consistent with all regional discharge planners and other Jail Authority facilities.
12. Must be able to safely operate a motor vehicle to ensure the delivery of community based services.
