The SUDP provides services to FPATH, FHARPS, and OCRP services. As a member of a multi-disciplinary team which includes Bachelor’s and Master’s level clinical staff and peers, the SUDP services to participants enrolled in FPATH, FHARPS, and OCRP services. The team provides outreach and engagement services to individuals who have been involved in the criminal justice system or who have been identified as high utilizers of hospital emergency services, resulting in their placement on the HCA referral list. FPATH works in close collaboration with OCRP, FHARPS, and Forensic Navigators to locate, engage and build rapport with identified individuals. FPATH will not duplicate services offered through health and behavioral health benefits provided under other programs but will leverage services otherwise available to enhance the services available to individuals on the referral list.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

1. Locate individuals identified on the HCA referral list.

2. Provide participants with SUD services and support including but not limited to:

  • Refer and connect participants to inpatient, outpatient, SUD prescribers, or other treatment programs

  • Provide individual and/or group SUD counseling sessions.

  • Act as a liaison to other SUD providers

3. Coordinate closely with Trueblood teams, act as a clinical consultant, and be a subject matter expert in evidence-based practices related to complex behavioral health conditions or co-occurring disorders including but not limited to:

  • Harm reduction.

  • Trauma-informed care

  • Person-centered approach

  • Contingency management

  • MOUD and

  • Motivational interviewing


4. Provide low barrier and accessible services including but not limited to participant housing, jails, hospitals, and other community-based settings.

5. Create or further develop recovery plans and on-going support to promote progress in care.

6. Provision crisis and hospital diversion services to individuals at high risk.

7. Complete suicide and homicide risk assessments to determine a level of response needed to maintain the safety of the client or others.

8. Assist non-Medicaid individuals to access Medicaid as appropriate.

9. Assist with obtaining SOAR services.

10. Provide evaluation and treatment for clients who have behavioral health concerns.

11. Assist and complete care plans and provide assessments and referrals for outpatient or inpatient treatment.

12. Provide consultation, training, and education to staff regarding behavioral health issues as assigned by supervisor.

13. Facilitate use of/access to crisis hospital diversion resources, including Stabilization and/or E&T admission.

14. Identify and include the client’s support network in care coordination.

15. Assist clients in identifying and accessing resources which will meet their needs.

16. Coordinate and collaborate with community resources on behalf of clients.

17. Maintain client records and all other required documentation in a timely and confidential manner, in accordance with agency policies and practices.

18. Requires travel to see clients in the community and travel between work sites either via FBH owned Vehicles and/or personal vehicle.

19. Other duties as assigned.

QUALIFIED APPLICANTS WILL HAVE:

REQUIRED:

  • Associate Degree in Addiction Studies or Substance Use Counseling or a related degree type.

  • DOH certification as a Substance Use Disorder Professional

  • Must apply for a Counselor Agency Affiliated – Registered credential from the WA State Department of Health within 30 days of hire.

  • Must have an unrestricted and valid driver’s license, be able to pass a motor vehicle screening and driving test, have active car insurance, and be willing to use their personal vehicle in the course of work.

PREFERRED:

  • Bachelor’s or Master’s degree in behavioral health related field

  • Two years of experience working with psychiatrically impaired populations

  • Experience in the criminal justice system

KNOWLEDGE, SKILLS & ABILITIES:

  • Ability to communicate and engage effectively with individuals of varied, backgrounds, trauma experiences, cultures, education level and socio-economic status.

PHYSICAL, SENSORY & ENVIRONMENTAL:

  • This position provides client services in community settings on a daily basis.

  • Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the community at large.

  • Some environments may not be ideal in terms of cleanliness or being habitable.

  • Daily travel between locations is required.

  • The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required.

  • Daily use of a computer is required.

  • May be required to lift 10 - 20 pounds.

  • Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens.

PAY RANGE (Hourly/ Non-Exempt): Depends on degree level as noted below:

  • Associate's Degree - $25.53 - $31.10

  • Bachelor’s Degree - $27.59 - $33.24

  • Master’s Degree or higher - $33.53 - $40.48

All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay for this position is as follows:

  • Associate's Degree - $25.53

  • Bachelor’s Degree - $27.59 per hour.

  • Master’s Degree or higher - $33.53 per hour.

Benefits described above are based on employees who hold .5 FTE positions or greater and/or some benefits vary based on FTE. PRN positions are not benefit eligible or eligible for a sign on bonus.
