Description

LCSW/LMSWs provide community-based integrated behavioral health services within the context of rural

primary care medical clinics: including assessment, diagnosis, and treatment of mental and emotional

disorders, including mental health and substance use conditions, health behaviors (including their

contribution to chronic medical illnesses), life stressors and crises, stress-related physical symptoms,

and ineffective patterns of health care utilization; psychoeducational techniques aimed at the

prevention of such disorders; consultation to individuals, couples, families, groups, and organizations;

collaborative care with other healthcare professionals; and research into more effective therapeutic

treatment modalities. 

Requirements

Duties and Responsibilities

1. Provides clinical services.

a. Maintains clinical expertise and competency across the life span as well as demonstrates

thorough knowledge of procedures to deliver quality behavioral health care.

b. Provides clinical social work services in scheduled and as needed clinic sessions, at each

assigned clinic, including extended hours.

c. Assesses, diagnoses, treats, and/or refers clients to specialty care services, as needed.

d. Will perform assessment for risk of harm to self and others and engage in appropriate safety

planning, treatment, and/or referral as necessary.

e. Ensures that client receives psycho-education regarding their diagnosis and the plan of care.

f. Collaborates with other healthcare professionals to provide integrated behavioral

healthcare services.

g. Provides continuity of care to clinic clients on his or her caseload and other clients as

assigned.

h. Maintains timely behavioral health documentation for each client seen in accordance with

the Center’s established Health Information Management policies and procedures to

include providing appropriate and legible documentation.

i. Comply with applicable privacy and confidentiality laws and regulations, to include, but not

limited to, the Health Insurance Portability and Accountability Act and Privacy Standards

(HIPAA) and the Texas Medical Privacy Act (Texas Health & Safety Code, Chapter 181). 

2. Provider will be familiar with PCMH Standards and work as a part of the medical team to perform

client centered care.

3. Attend staff meetings and required trainings as needed.

4. Promote and maintain effective community relationships.

5. Maintains professional courtesy at all times.

6. Performs all other related duties as assigned.

 Knowledge, Skills and Abilities Required

• Knowledge of the Center’s policies and procedures and an ability to obtain specific knowledge of the Center’s current operational protocols 

• Knowledge of medical terminology, medical conditions, medical treatments, and appropriate use of approved abbreviations 

• Ability to document, with clarity, all information relevant to a client’s needs and care provided 

• Ability to effectively present behavioral health issues in small groups, to clients, Center staff, stakeholders, and the general public

• Ability to demonstrate use of electronic communication devices, including but not limited to computers, fax machines, telephones, and copiers while maintaining HIPAA compliance 

• Proven interpersonal skills and effective as a member of a team 

• Knowledge and experience in identification and reporting of abuse, neglect, or exploitation of children, the elderly, or persons with disability; 

Preferred 

• Bilingual (English and Spanish) and able to provide services in Spanish, both verbal and written.

Credentials and Experience

Required 

• Master’s degree in social work and current Texas LCSW

• One year of practice experience as an LCSW, performing diagnosis and treatment of mental illness 

• Current Cardiopulmonary Resuscitation (CPR) for Healthcare Professionals Certification 

• Must pass comprehensive background check

Preferred 

• 2 years of medical social work or medical case management experience 

• Training in suicide prevention and crisis intervention 

• Experience with Integrated Behavioral Healthcare in a primary care setting 

Special Requirements 

The employee must be able to lift and/or move more than 30 pounds. Requires sitting, standing and walking for extended periods of time. Flexibility to work under stressful conditions and extended hours, at times.
