The duties listed below are essential functions of the position that must be performed to effectively perform the position in at least an acceptable manner as defined by Front Porch. In the future, other duties may be assigned and considered essential functions of the position:
- Assesses residents upon admission and weekly, as well as when their condition changes, and prescribes appropriate nursing action.
- Notifies physicians of changes in resident condition and follows through until appropriate action is taken.
- Implements the resident's plan of care based on assessments and goals established by the Interdisciplinary Team.
- Documents changes in residents' condition and nursing care in the appropriate system(s), including progress notes and an on-the-care plan, updating the care plan as needed.
- Administers medications and performs treatments according to the physician's policies and procedures.
- Oversees other members of the health care team as needed to accomplish the plan of care.
- Functions as a resident advocate by protecting and ensuring resident rights.
- Consistently involves residents and families in planning care, treatment, and teaching needs.
- Coordinates and performs needed treatments, medications, or other nursing interventions as indicated by the patient's care plan or as ordered by the physician.
- Follows proper infection control procedures and supervises staff in infection control procedures.
- Maintains inventory of medical supplies and medications to meet patient/resident needs.
- Perform all other related duties as assigned.
Essential:
- Licensed Vocational Nurse - California
- First Aid certificate
Education with Equivalent Qualifications:
- 2-year College or technical school
Experience with Equivalent Qualifications:
- 1 Year RN/LVN experience providing direct care
