Job Summary

We are seeking a compassionate and dedicated In Home Caregiver to provide exceptional support and assistance to individuals in their own homes. Responsibilities

• Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, and personal hygiene.
• Support patients with mobility and transfer methods using equipment such as Hoyer lifts or patient transfer techniques to ensure safety and comfort.
• Monitor vital signs and observe patient conditions closely, reporting any changes or concerns promptly to healthcare professionals.
• Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation.
• Administer medications accurately following care plans while adhering to HIPAA regulations to protect patient privacy.
• Provide companionship and emotional support, fostering a positive environment that encourages social interaction and mental well-being.
• Maintain detailed documentation of care provided, patient progress, and any incidents in compliance with home health standards and regulations.
• Support clients during medical appointments or community outings by providing personal transportation assistance when needed.

Requirements

• Proven experience working as a caregiver in home health, assisted living, nursing homes, or group home settings.
• Knowledge of patient observation techniques, dementia care (including Alzheimer’s and memory care), and neurodevelopmental disorders such as autism.
• Ability to perform manual handling tasks safely, including heavy lifting and the use of patient transfer equipment like Hoyer lifts.
• Familiarity with medication administration, vital signs monitoring, CPR/First Aid certification, and basic nursing skills.
• Strong communication skills to effectively interact with clients, families, healthcare teams, and social workers.
• Experience working with individuals with developmental disabilities or intellectual disabilities is highly valued.
• Understanding of HIPAA compliance and confidentiality standards related to home health care.
• Ability to perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment for clients.
• Valid driver’s license or reliable transportation for client outings or appointments is preferred but not mandatory.
