About Us
From The Heart Home Care is launching a new Outpatient Hospice Program serving communities across South Carolina. We are seeking a compassionate, licensed Hospice Social Worker to join our Interdisciplinary Team (IDT). This is a PRN/part-time position with flexible scheduling and light workload during start-up.
Position Summary
The Hospice Social Worker provides emotional, psychosocial, and resource-related support to hospice patients and their families. The Social Worker conducts assessments, participates in care planning, supports families through end-of-life issues, and coordinates community resources. This role works closely with the Medical Director, Administrator, RN Case Manager, Chaplain, and Volunteer Coordinator.
Responsibilities
- Complete initial psychosocial assessments for hospice patients and families.
- Participate in Interdisciplinary Team (IDT) meetings (virtual options available).
- Develop individualized social work care plans and update as needed.
- Provide counseling, grief support, adjustment assistance, and crisis intervention.
- Assist families in accessing community resources, benefits, and support programs.
- Serve as a patient and family advocate while coordinating with healthcare partners.
- Maintain accurate, timely documentation according to hospice standards.
- Collaborate with nursing, chaplaincy, and leadership to support the patient’s plan of care.
- Complete required hospice orientation and annual trainings (HIPAA, OSHA, etc.).
- Ensure compliance with all SC DHEC and hospice regulations.
Qualifications Required:
- Bachelor’s or Master’s Degree in Social Work
- Licensed Social Worker in South Carolina (LMSW or LISW-CP)
- Experience in hospice, end-of-life care, medical social work, counseling, or geriatrics
- Strong communication, documentation, and assessment skills
- Ability to work PRN with flexibility
- Must pass background check and TB screening
Preferred:
- Prior hospice or palliative care experience
- Familiarity with EMR documentation
- Knowledge of SC community support programs, Medicaid/Medicare, and bereavement needs
Schedule
- PRN / Part-time
- Flexible hours
- Virtual IDT meetings
- Occasional home visits may be required (rare in outpatient start-up phase)
Compensation
- Competitive PRN rate
- Paid per assessment, documentation, and IDT meeting (Rate based on experience)
- Compensation structure includes:
- $75 – $125 per completed psychosocial assessment
- $50 – $80 per IDT meeting (virtual)
- $30 – $45 per hour for training/orientation (limited)
Final rates are based on experience, licensure level (LMSW or LISW-CP), and availability.
