We are seeking a dynamic, relationship-driven Home Care Community Liaison to join our growing team. This role is perfect for a motivated professional who thrives on building connections, educating the community, and generating meaningful relationships with seniors.
This role is based in-person (75% in the field / 25% in the office)
North Bay Territory includes: Sonoma County & Marin County

Compensation & Benefits:
- Salary $80,000 per year Commission

Responsibilities:
- Build and maintain relationships with referral sources including hospitals, rehab centers, senior living communities, case managers, and other healthcare professionals
- Represent our agency at networking events, health fairs, and community forums
- Meet with seniors and their loved ones to educate them on home care and ensure they are receiving quality care.
- Work closely with Care Coordinators to ensure smooth intake processes, including processing intakes and conducting new client interviews.
- Track outreach activity and results using CRM and reporting tools.
- Maintain consistent communication with leadership regarding territory performance and opportunities.
Qualifications/Requirements:
- Must have at least 2 years of home care sales experience in the field, combining client assessments, referral source visits, and client onboarding.
- Excellent communication and relationship-building skills.
- Self-starter with strong organizational and time management abilities.
- Comfortable working independently in the field / over-communicating.
- Valid driver’s license with clean driving record (company-provided vehicle available)
