Job Summary:
The Ambulatory Transplant Pharmacist is a designated member of the transplant team and is assigned primary responsibility for providing comprehensive pharmaceutical care to organ transplant (heart, liver, lung, kidney, and pancreas) recipients. The pharmacist evaluates, identifies and solves medication related problems for transplant recipients and is a liaison between patients and patients' families and other health care team members regarding medication issues. They work with patients and their families and members of the transplant team including physicians, nurses, and coordinators to contribute to the continuity of care for each transplant patient. They also collaborate with physicians, nurses and other healthcare professionals to ensure the delivery of safe, effective, and cost-efficient medication treatment. This pharmacist will be part of the ambulatory clinical pharmacy team within the transplant clinics. The pharmacists provide direct patient care via in-person office visits, virtual visits, and telephone encounters and manage disease states through collaborative practice agreements. Other duties as assigned.
EXPERIENCE DESCRIPTION:
A minimum of one year of hospital or three years of other pharmacy experience is preferred.
EDUCATION DESCRIPTION:
Bachelor's or Pharm. D. degree from an ACPE certified school of pharmacy is required. Successful completion of an ASHP accredited PGY1 residency is required. Successful completion of an ASHP accredited PGY2 residency and/or work experience (at least 2 years) with the transplant population is strongly preferred.
SPECIAL SKILLS DESCRIPTION:
Demonstrates effective use of computers and applications (e.g., MS Word, Excel, Outlook) as appropriate to practice and communicate effectively. Active membership in national organization appropriate for specialty.
LICENSURE DESCRIPTION:
Active Wisconsin pharmacist license (or eligible for) is required.
