Reports to: Case Manager Supervisor

Category: Full-Time, Non-Exempt

Location: Washington, DC 20009

Hours: Flexible schedule, Monday through Friday, between 7 AM-8 PM. Some, holidays, and weekends may be required to support operations and to respond to emergencies.

Our programs are person-centric and individualized to meet the needs and goals of individuals, families, youth/young adults, and Veterans experiencing homelessness. We provide outreach, hospitality, health care, case management, housing, job placement services, and advocacy. Summary: The position provides permanent supportive housing services to individuals experiencing homelessness, identified as the most vulnerable in the District of Columbia. The Neighbors First Program follows a Housing First model and philosophy. The Case Manager works to empower participants to rebuild their lives, relying on their strengths and using wraparound resources in a holistic, participant-driven process.


Responsibilities:

  • Provide supportive services to individuals experiencing homelessness with serious mental illnesses, addictions, medical concerns, etc.
  • Deliver face-to-face contact to consumers, spending approximately 80% of the time in the field
  • Demonstrate empathy and flexibility in responding to consumers’ needs and requests, fostering wellness and independence
  • Engage consumers and assess individual needs, preferences, and strengths
  • Develop, implement, and revise individual service plans
  • Assist households in locating and selecting wraparound services
  • Monitor medical/psychiatric needs and medication management of all household members
  • Assist participants in securing adequate housing resources
  • Act as a liaison with landlords and other community stakeholders
  • Participate in multidisciplinary meetings and regular supervision
  • Collect outcome statistics and write monthly progress reports
  • Maintain participant’s electronic records in the Housing the Homeless database
  • Attend professional training and conferences as needed
  • Perform all other duties as assigned.

Requirements

Education:

  • A High School diploma or GED and six years of direct human service experience providing counseling and/or case management services to vulnerable or at-risk populations OR
  • A Bachelor’s degree in any field and two years of direct human service experience providing counseling and/or case management services to vulnerable or at-risk populations OR
  • A Master’s degree in Social Work or a related human services field and one to two years of experience
  • Must have or be able to obtain a First Aid/CPR/AED certification that includes a hands-on skills testing module.? May be called into action to perform life-saving measures. First Aid/CPR/AED training will be provided by Friendship Place

Qualifications: 

  • Demonstrated professional knowledge of the theories, principles, techniques, and practices of social service delivery systems
  • Understanding of adult rehabilitation services
  • Experience with families, youth, and school systems
  • Experience with addictions, mental health, and co-occurring disorders
  • Strong written and verbal skills
  • Must have strong intra- and interpersonal communication skills, with the ability to communicate professionally and effectively both in writing and verbally.
  • Ability to work in a culturally diverse environment
  • Ability to work well under pressure
  • Ability to work as a team player
  • Strong commitment to mission, including racial equity and social justice

Physical and Environmental Requirements: The incumbent will spend time between office/desk settings, traveling to meet with households across the service area, and attending meetings/engagements with key stakeholders. Field time consists of up to 80%. Employees must be able to sit for extended periods with repetitive motion and positions. The employee must be able to operate a motorized vehicle. Some lifting may be required (up to 20-25lbs)


Background Checks: This position is classified as Protection Sensitive under DHS. The selected candidate will be required to complete and pass background checks specific to contract requirements that may include criminal history background check and fingerprinting, FBI Background Check, DC Metropolitan Police Background Check, State-of-Residence Police Background Check, DC Child Abuse Registry (CFSA), National Sex Offender Registry Check, Drug Panel and Alcohol Screening, Tuberculosis Test, and Driving Record check (for positions transporting participants). Reference checks and verification of current and/or previous employment are required.

Specific Requirements: Must have reliable transportation to carry out job duties and responsibilities. The vehicle must meet legal requirements (insured, registered, etc.), and the employee must have a valid driver's license

Compensation: The hiring pay range is between $58,117.00 - $63,506.00. The starting annual salary is based on a step system. A candidate’s starting pay determination is made based on meeting the basic position requirements, and current completed education, credentials, and work experience. Please Note: The step system is set and not open to pay negotiation.
