Blended Case Manager (BCM), will promote resiliency and recovery for children, adolescents, adults and their families by accessing, coordination, mentoring, and advocating for community resources and services.
Qualifications:Bachelor’s Degree in Human Services or related field. Must work 40 hours per week, have flexible schedule, and on-call is required. Blended Case Management experience preferred.
