What are the responsibilities and job description for the RN/LPN All Shifts Full Time and Part Time Available position at FRIENDSHIP HAVEN?
Purpose of the Position
To provide a broad range of Nursing services to meet the individual needs of each resident’s social, environmental and personal care in the resident’s home to enhance the capacity of residents to attain or maintain their highest level of independence, offering choice and control.
Responsibilities
- Communicates and documents services and care in an effective, timely manner in accordance with care plan and policies and procedures. Passes information on to co-workers to provide continuity of services.
- Adapts a proactive approach to resident care anticipating the resident’s needs.
- Accommodates resident needs by responding to each resident’s expression of needs and adapts approach to meet resident’s behavioral needs.
- Safeguards residents’ property.
- Communicates effectively and appropriately with physicians, medical professionals and family members regarding resident’s condition.
- Individualizes services to meet the unique needs of each resident. Maintains resident’s self-esteem, privacy and confidentiality of personal information. Minimizes resident losses, maximizes abilities and strengths.
- Provides residents with opportunities for independence and choice consistent with their service plans, involves residents in self care activities. Promotes independence and wellness.
- Assures resident rights are respected and protected.
- Works with team members and other staff members in a helpful, respectful and courteous manner.
- Reports indirectly to the DON who completes scheduling for all nursing staff along with scheduling mandatory meetings and skills training.
- Responds appropriately and timely to resident requests for help and maintain positive relationships with resident’s friends and family members. Creates and maintains an atmosphere of warmth and personal interest, as well as a calm environment.
- Treats residents with unconditional positive regard; focuses on resident’s strengths; attempts to support or minimize weaknesses; looks at underlying causes of behavior.
- Follows general safety, first aid, emergency response and infection control procedures.
- Assures physician’s orders are processed and physicians notified in a timely manner.
- Meets requirement for in-service training and meeting attendance per campus policy and state and federal requirements. This also includes maintaining required licenses and/or certifications.
- Admits, transfers and discharges residents as necessary.
- Participates in and develops written and comprehensive nursing assessments of the nursing needs of each resident as required by campus policy and regulatory requirements.
- Assists Caregivers with resident cares and rounds, as needed.
- Responsible to ensure Caregivers are following policies and providing resident cares per the noted care plan. If a Caregiver is not, will be asked to follow the Friendship Haven progressive discipline policy by writing a formal counseling or warning for the individual. Any violations should also be reported to the Clinical Coordinator.

Minimum Requirements
· Education: Current unencumbered license as RN or LPN.
· Experience: Working in aging related environments preferred, but not required.
· CPR Certification.
Physical Demands
· Sits, stands, bends, lifts and moves during work hours.
· Willing to work beyond normal working hours, and on weekends and holidays when necessary.
· Should be capable of lifting up to 50 pounds on a regular basis.
Mental Demands
· Subject to frequent interruptions.
· May be subject to hostile and emotionally upset residents, families and visitors.
