Assisted Living Nurse Manager

Supervised by: Director of Assisted and Independent Living

Purpose of the Position:

Participates in assessing, planning, organizing, directing, delegating and facilitating the clinical care and services provided to the general population of Assisted Living along with Friendship Haven’s private home care. This position assesses and evaluates functional abilities, health history and cognitive status of residents and participants. For residents who require personal and health related care and services this position is responsible for the evaluation, coordination, supervision and delegation of personal and health related services and care provided by staff. Responsibilities include selection of employees, staff training and education, delegation of services and ensuring the competency of staff providing personal and health related services and care.

Essential Functions:

· Performs nurse delegation and assures health and personal care provided in Assisted Living is consistent with Iowa Code Chapter 231C and Administrative Rules Chapters 6, 67, 69 and 70, and other regulations therein referenced in order to maintain state certification.

· Performs nurse delegation of health and personal care provided in Assisted Living and under Second Family Home Care.

· Oversight of care delivery and reviewing to ensure care meets standards of practice.

· Provides case management to assure Assisted Living residents are receiving the services they need and are appropriate for continued assisted living occupancy.

· Participates in campus wide care plan development and transitions of residents across the continuum of care.

· Provides nurse review of Second Family clients receiving delegated services no less than quarterly and makes referrals of issues as needed.

· Follows Friendship Haven policies and procedures.

· Participates in the development, implementation and review of policy and procedures specific to the assisted living program.

· Coordinates team member interviews and the selection of new staff to ensure that quality and services are provided to residents.

· Serves on committees, as requested.

· Supervises direct care givers and services provided by direct care givers in assisted living.

· Conducts resident/participant evaluations, service plans and nurse reviews within appropriate time frames as needed.

· Participates in resident and family orientation.

· Addresses the health and personal care needs of residents and keeps family members informed.

· Provides education to residents and families related to health care and aging issues.

· Develops education and training schedule for team members and provides education to team on health care, personal cares, and age-related topics.

· Plans and conducts assisted living team monthly meetings.

· Participates in the planning and development of the operating budget and maintains fiscal accountability in areas related to nursing expenditures and supplies. Participates in the development, implementation and evaluation of strategic plans, goals and objectives for assisted loving program and private home care.

· Works closely with the Assisted Living Leadership Team to evaluate and assess delegations, household processes, and assisted living program as a whole, to promote a positive cohesive team through leadership and mentoring of all assisted living staff.

· Provides oversight and makes necessary requests in order to preserve the physical appearance of the neighborhoods.

· Responsible to communicate with Medicare providers on the resident’s behalf.

· Provides fall/incident reviews and follow up as necessary.

· Participates in a weekend on-call rotation providing nursing services to the assisted living program.

Knowledge, Skills and Abilities:

· Knowledge of a social model of service delivery resident centered care.

· Knowledge and understanding of Iowa Code Chapter 231C and Administrative Rules Chapters 6, 67, 69 and 70.

· Knowledge of aging issues.

· Support the mission, vision and values of Friendship Haven.

· Abide by Friendship Haven Standards of Personal Performance and Code of Ethics.

· Ability to work effectively with co-workers in a team environment.

· Proficiency in writing and public speaking.

· Proficiency in computer usage and related applications.

· Ability to self-initiate with minimal supervision.

· Demonstrate strong organizational skills.

· Ability to deliver excellent customer service and public relations.

Minimum Requirements:

· Education: Current license to practice as a Nurse in the State of Iowa

· Licensure: RN or LPN required.

· Experience: Experience in a supervisory capacity in an assisted living/health care setting.

· CPR Certification, or the ability to become certified.

Physical Demands:

· Sits, stands, bends, lifts and moves intermittently during work hours.

· Willing to work beyond normal working hours, and on weekends and holidays when necessary.

Mental Demands:

· Subject to frequent interruptions.

· May be subject to hostile and emotionally upset residents, families and visitors.
