Duties
- Monitor clients health status by observing vital signs, patient behavior, and overall condition throughout the night
- Assist with activities of daily living (ADLs) such as toileting, bathing, dressing, and mobility support using equipment like Hoyer lifts when necessary
- Medication reminders following care plans and HIPAA confidentiality standards while maintaining accurate documentation
- Prepare nutritious meals and snacks tailored to individual dietary needs, ensuring proper meal timing and hygiene standards
- Conduct thorough patient observations to identify changes or concerns that require immediate attention or escalation
- Maintain a clean, safe environment by performing light cleaning duties and ensuring all safety protocols are followed
- Support residents with dementia, Alzheimer’s, autism, or developmental disabilities through specialized memory care techniques and compassionate engagement
- Follow emergency procedures including CPR certification protocols in case of medical emergencies or accidents
Experience
- Prior experience in home health care, assisted living facilities, nursing homes, or group homes is highly preferred
- Demonstrated knowledge of patient monitoring and vital sign measurement
- Familiarity with care plans development and implementation for individuals with complex needs such as dementia or developmental disabilities
- Hands-on experience with patient transfer equipment like Hoyer lifts and resident mobility aids
- Strong understanding of HIPAA regulations to ensure confidentiality and privacy of patient information
- Experience working with individuals with Seizures, Alzheimer’s disease, or other memory impairments is advantageous
- Ability to perform heavy lifting safely and effectively while maintaining resident comfort and safety
- Certification in CPR and first aid is required; additional certifications in hospice care or nursing are a plus
