Position Summary:

Under the supervision of the Meaningful Day Services Director and in concert with Friends Aware’s policies and procedures and relevant federal, state and local regulations, the Case Manager’s responsibility is to oversee various aspects of service delivery to individuals served. The primary duties include the development of the Person-Centered Plan (PCP) for Meaningful Day Services for designated individuals served, case management, assist individuals in securing needed services and advocate on their behalf, staff training, the maintenance of files for individuals served, along with other day to day to service needs. Services to individuals are facility and community based and may include services as designated in the PCPs. This is a non-exempt position with a flexible schedule of 40 hours a week.

Major Areas of Responsibility:

Maintains proper medication administration forms/records for the distribution of medication to individuals served.

  • Works cooperatively as a team member in all aspects of the Meaningful Day Services.
  • Acts as the Case Manager for designated individuals to assist with goal planning, goal monitoring, goal revisions, communication with families/caregivers Coordinators of Community Services, etc.
  • Acts as a liaison between staff, nurses, and other agencies regarding consumer health issues for designated individuals. Signs off on nursing assessments.
  • Ensures updated Person-Centered Plan (PCP) staff training along with individual specific staff training for individuals served.
  • Provides staff training on Intro to DD and Abuse Prevention.
  • Oversees lunch time routines for individuals and staff to ensure proper staffing/assistance, adherence to special dietary needs of individuals served, ensures staff follow proper procedures upon a dietary incident, etc.
  • Maintains updated and organized files for individuals served.
  • Completes area observations and checklist to assist in providing regular feedback and performance monitoring of staff.
  • Provides training, mentoring and modeling to direct care staff in the provision of services to individuals served.
  • Observes/mentors and/or assists with training of staff in the MDS Program and provides counseling/demonstration on appropriate interactions with individuals or training methods.
  • Assists, when needed, in group facilitation and/or covering areas when additional staffing is needed to meet staffing ratios.
  • Reports to the Meaningful Days Services Resource Manager/Meaningful Day Services Director the quality of staff interactions.
  • Immediately or as soon as is feasible, report to the Meaningful Days Service Resource Manager/Meaningful Day Services Director any safety and/or health issue on behalf of staff and individuals served.
  • Assumes necessary duties, in the absence of the Meaningful Day Services Resource Manager and Director, to provide day program services without interruption including involvement in crisis situations.
  • Assists the MDS Resource Manager and MDS Director with the planning and coordination of special projects including but not limited to grants, adaptive equipment, adaptive/augmentative communication, computer software, AV equipment, staff training, etc.
  • Ensures staff are documenting in the individual’s diary pertaining to the specific issues that do not require an incident report.
  • Advocates for individual’s human rights in all program decisions.
  • Maintains confidentiality in all matters related to the individual’s program and personal circumstances.
  • Assists program with Quality Assurance, Admissions Committee, Employment Recognition and Retention, Friends for Life activities.
  • Actively participates in staff meetings and attends staff training.
  • Delivers meaningful day services to individuals as required and recommended in their PCP in the least restrictive treatment model.
  • Attends PCP meetings to review the individual’s progress and current level of support needed in a professional manner.
  • Utilizes appropriate lines of authority to address personnel issues, concerns, questions, etc.
  • Helps in directing individuals to make responsible choices regarding their individual rights in all aspects of their program.
  • Displays sound judgment in exercising responsibility for the safety and wellbeing of the individuals and maintain a safe work/training environment at all times.
  • Performs other duties as assigned by Supervisor.

While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time. The noise level in the work environment is usually low to moderate. The on-site and off-site meetings, conferences, appearances, seminars, etc. attended by the incumbent occur in a typical office environment.

The employee must occasionally lift and/or move more than 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

Qualifications:

1. Ability to read and interpret regulations.

2. Good verbal, written, and interpersonal skills.

3. Good time management and organizational skills.

4. Ability to learn and effectively use software applications within agency-designated timeframe.

5. Must possess typing skills sufficient to be productive.

6. Valid driver’s license, with no more than 3 points on record.

7. Must be at least 19 years of age.

Education and Work Experience:

High School Diploma or GED and a minimum of 2 years experience in the field of developmental disabilities.

CMT Certification must be obtained within 90 days of hire.

DOT Certification must be obtained within 90 days
