Registered Nurse (RN) Case Manager Home Health

What You’ll Do

• Conduct thorough patient assessments and create individualized care plans.
• Coordinate and oversee patient services, ensuring compassionate, quality care.
• Educate and support patients and families on health management and discharge planning.
• Collaborate with an interdisciplinary team to optimize outcomes.
• Monitor progress, update care plans, and adjust to changing needs.

What We’re Looking For

• Current RN License (in good standing).
• Experience in hospital, critical care, or discharge planning preferred.
• Strong knowledge of patient assessment, care coordination, and managed care.
• Familiarity with geriatrics and post-acute care.
• Excellent communication and patient relationship skills.
