We are seeking a dynamic and experienced Director of Patient Care for Home Health to lead and oversee the delivery of exceptional nursing services within a home health setting. Responsibilities
- Lead the development and implementation of comprehensive care plans tailored to each patient's needs, ensuring alignment with clinical best practices and regulatory standards.
- Supervise and mentor nursing staff, including nurse managers and case managers, fostering professional growth and maintaining high levels of team engagement.
- Oversee patient monitoring activities, ensuring timely assessments, documentation review, and accurate medical records management using Electronic Medical Record (EMR) systems.
- Manage shift scheduling, staffing levels, and resource allocation to ensure seamless service delivery while maintaining compliance with HIPAA privacy regulations.
- Collaborate with interdisciplinary teams—including hospice care providers, assisted living facilities, and physicians—to coordinate care transitions and optimize patient outcomes.
- Monitor quality metrics related to Medicare requirements, care documentation accuracy, and patient satisfaction scores; implement continuous improvement initiatives accordingly.
- Oversee budget management related to staffing, supplies, and operational expenses to maintain financial health while delivering top-tier care.
Requirements
- Proven experience in critical care, home health services, assisted living environments, hospice care or acute care settings with demonstrated leadership capabilities.
- Supervising experience in managing nursing teams within home health or similar healthcare organizations.
- Strong knowledge of physiology, anatomy, medical terminology, and nursing practices essential for effective case management.
- Familiarity with EMR systems for documentation review and medical records management; proficiency in shift management and scheduling tools.
- Deep understanding of HIPAA regulations to safeguard patient privacy; experience with Medicare guidelines is highly desirable.
- Excellent leadership skills with the ability to motivate teams, foster collaboration, and drive quality improvement initiatives.
- Ability to analyze clinical data to develop appropriate care plans; experience in budgeting and resource planning is advantageous.
