What are the responsibilities and job description for the Support Planner position at FRESH START THERAPEUTIC SERVICES LLC?
The Support Planner is responsible for providing person-centered case management services to individuals enrolled in Medicaid Long-Term Services and Supports programs. This role involves assessing applicants’ and participants’ needs, strengths, and preferences; developing and monitoring individualized Plans of Service (POS); coordinating community-based services; and ensuring compliance with program requirements and documentation standards. The Support Planner maintains ongoing communication with participants, providers, and interdisciplinary team members to support the participant’s goals, promote independence, and improve quality of life. This is a hybrid position requiring both in-person visits and remote documentation within required timeframes. 8:30am-5:00pm
