The Licensed Medical Social Worker will play a vital role in supporting patients and their families by providing emotional and social support, assisting with the challenges of chronic kidney disease, and helping coordinate care plans. Key responsibilities include patient advocacy, conducting psychosocial assessments, developing individualized care plans, providing counseling, and assisting with resource referrals. This is a full-time, on-site position located in San Diego, CA.
- Strong skills in conducting assessments, counseling, and providing social support
- Proficiency in case management and individualized care planning
- Experience with patient advocacy and resource coordination
- Knowledge of chronic illnesses and healthcare systems, particularly regarding renal disease
- Clear and effective written and verbal communication skills
- Ability to work collaboratively as part of a multidisciplinary team
- Current licensure as a Clinical Social Worker (LCSW) or equivalent in the state of California
- Master’s degree in Social Work (MSW) or related field required
- Experience in healthcare or medical social work is highly desirable
