What are the responsibilities and job description for the PHP Counselor position at Freeman?
Counselor
Primary Responsibilities:
- Assess each potential client’s needs, service availability, and appropriateness for FRC programs
- Perform clinical assessments on new admissions
- Provide individual and group counseling, including therapeutic interventions designed to foster each client’s recovery from substance use disorder and overall wellbeing
- Work with the Clinical Team to meet the meet each client’s needs and individual goals
- Provide the appropriate documentation for services including treatment goals, progress notes, and any other documentation needed
- Guide patients in the development of helpful skills and wrap-around recovery services and supports
- Provide case management to clients to help them access all needed resources
- Assist Clinical Supervisor and Clinical Manager in any operational or administrative tasks
Reports To: Clinical Supervisor
Requirements:
- Licensed or license-eligible under the requirements of the State of Tennessee (LADAC II, LADAC I, LMSW, LCSW, LPC-MHSP, LMFT, etc.)
- Excellent written and verbal communication skills
- May required CPR, First Aid/AED certification
