Job Overview:
As RN's & LPN's at Freedom Plaza Care Center, you will be an essential part of a dedicated care team providing compassionate, person-centered care to residents in a Skilled Nursing Facility (SNF) environment. This role goes beyond clinical tasks—it's about forming meaningful relationships with residents, ensuring their dignity and quality of life, and working collaboratively in a close-knit, supportive team.
You’ll be responsible for delivering nursing care to individuals who are recovering from surgery, illness, or injury, as well as to those requiring long-term, chronic care management. Our residents may have a variety of complex needs, including rehabilitation, wound care, medication management, or memory support. Your ability to assess, respond, and provide thoughtful, skilled care is vital to improving health outcomes and ensuring comfort and safety.
Key Responsibilities:
- Provide direct nursing care to residents in accordance with their care plans
- Monitor patients' health status and promptly report any changes
- Administer medications and treatments as prescribed by healthcare providers
- Assist with admissions, discharges, and care plan updates
- Collaborate with interdisciplinary teams including CNAs, therapists, and physicians
- Maintain accurate, timely documentation in compliance with facility policies and state/federal regulations
- Support residents and their families with education and guidance regarding ongoing health needs
- Foster a positive, respectful, and safe environment for residents and staff
What you Bring to the Team:
· Certification: Current and valid RN or LPN license in the state of Arizona
· Communication Skills: strong interpersonal skills to work effectively with patients, families, and interdisciplinary team
· Experience: Previous experience in a healthcare setting is a plus
· Teamwork: Ability to work effectively in a team-oriented environment
· Organization: Strong organizational and time management skills
