Job Title: Home Health Registered Nurse
Job Responsibilities
- Conduct initial assessments and develop individualized care plans for clients
- Provide skilled nursing care to clients in their homes, including medication management, wound care, and symptom management
- Coordinate with other members of the care team to ensure comprehensive and integrated care for clients
- Educate clients and their families on proper self-care techniques and health maintenance strategies
- Monitor and document client progress and communicate any changes or concerns to the physician
- Act as an advocate for clients and ensure their needs are met in a timely and respectful manner
- Adhere to all company policies, procedures, and regulatory requirements
- Active RN license in the state of Nebraska
- Minimum of 2 years of nursing experience, preferably in home health or a similar setting
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and collaboratively as part of a team
