Job Summary
As a Home Health Registered Nurse (RN), you will provide skilled nursing care to patients in their homes through individualized care plans developed in partnership with the Director of Home Health Operations and interdisciplinary care team. This is a full time position that would participate in a rotating On-Call schedule.
In this role, you will assess patient needs, coordinate services, educate patients and families, and help achieve positive clinical outcomes. Most importantly, you'll empower patients to maintain their independence and enjoy the highest possible quality of life.
Qualifications
- Current, unencumbered Iowa Registered Nurse (RN) license required
- Expereince in Home Health preferred
- Valid driver's license, reliable transportation, and current auto insurance
- CPR Certificate or ability to obtain
- Current Iowa Dependent Adult Abuse Mandatory Reporter Training or ability to obtain
- Ability to work independently while collaborating effectively with the interdisciplinary care team.
