Job Title/Position: Interdisciplinary Group Coordinator
Reports To: Hospice DON
JOB DESCRIPTION SUMMARY
The interdisciplinary group coordinator ensures the compliance of the IDG meeting.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITY
1. Prepares and organizes any documentation related to the IDG meeting.
2. Attends IDG meetings and takes an active role in the meetings.
3. Creates IDG meeting in EMR prior to each IDG meeting.
4. Ensures that the plan of care is coordinated, established, and reviewed before the IDG meeting.
5. Participates in QAPI activities.

POSITION QUALIFICATIONS
1. Bachelor's degree in the medical field preferred.
