Freedom Behavioral Hospital is a 82-bed acute care facility providing specialized care for adults with their mental health and behavioral health needs. The hospital offers a wide range of services, including inpatient and outpatient care, as well as a variety of specialized programs and is a trusted resource for those seeking quality care and support.
The LPN provides direct and indirect patient care services that meet the psycho-social, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a Registered Nurse. Communicates with physicians and team members about changes in patients' conditions, including results of diagnostic studies and symptomology.
Shifts are 12 hours with a 2/3 split rotation
We are currently hiring for the West Monroe facility.
RESPONSIBILITIES:
- Utilizes assessment scales to identify basic needs of patients by collecting data and identifying deviations from normal.
- Effectively communicates pertinent assessment findings with other health care members.
- Accurately and thoroughly document all findings in the patient record.
- Identifies expected outcomes based on assessment data.
- Implements nursing care interventions identified in the individualized multidisciplinary treatment plan using accepted standards of practice in collaboration with other health care providers.
- Performs nursing procedures/treatments as prescribed by physician.
- Performs vital signs/measurement on patients as ordered and as needed based on nursing judgment and documents same in the medical record.
- Administers medications utilizing established safety procedures / standards.
- Accurately communicates/documents medication doses prescribed.
- Transcribes orders accurately and performs chart audits of transcription accurately and according to facility guidelines.
- Follows up on patient care responsibilities; communicates incomplete assignments and patient needs with other disciplines to assure continuity of care.
- Performs other duties and projects as assigned.
PREFERRED KNOWLEDGE, SKILLS, AND/OR EXPERIENCE REQUIRED:
Education: Certification from approved Practical Nursing Program
License: Licensed as an LPN to practice in the State of Louisiana.
Experience: One year LPN experience preferred. Previous experience with behavioral health and/or geriatrics strongly preferred.
