Responsibilities
Evaluate patients in their homes and develop individualized treatment plans.
Provide skilled occupational therapy services to improve independence and quality of life.
Assess home safety and recommend adaptive equipment or modifications.
Educate patients and caregivers on therapeutic exercises, self-care techniques, and safe use of assistive devices.
Complete OASIS assessments and clinical documentation accurately and on time.
Collaborate with physicians, nurses, and other healthcare professionals to achieve the best patient outcomes.
Monitor patient progress and adjust treatment plans as appropriate.
Promote safe, compassionate, high-quality patient care.
Qualifications
Graduate of an accredited Occupational Therapy program.
Current Michigan Occupational Therapist license.
Current CPR certification.
Minimum of 2 years of Occupational Therapy experience preferred.
Home Health experience is a plus but not required.
Valid driver's license, reliable transportation, and auto insurance.
