Per Diem Physical Therapist (PT) – Outpatient Home Visits

Franklin Physical Therapy is growing and looking for a motivated, patient-focused Physical Therapist to provide outpatient physical therapy services in patients’ homes throughout Sussex County, New Jersey.
This is an excellent opportunity for a PT looking for flexible supplemental income, a better work-life balance, or the freedom to build a schedule around their existing commitments.
This is outpatient physical therapy delivered in the home — not traditional home health. Focus on individualized evaluations, skilled treatment, functional mobility, and meaningful patient outcomes without the typical Medicare Part A start-of-care workload.

Responsibilities
- Perform physical therapy evaluations and develop individualized plans of care
- Provide skilled therapeutic interventions in patients’ homes
- Treat patients with orthopedic conditions, balance deficits, weakness, mobility limitations, and other musculoskeletal impairments
- Progress patients toward functional goals and improved independence
- Complete timely, accurate clinical documentation
- Communicate effectively with patients, caregivers, and other healthcare professionals as needed
Qualifications
- Current New Jersey Physical Therapist license in good standing
- Strong clinical reasoning and communication skills
- Ability to work independently and manage visits responsibly
- Reliable transportation and willingness to travel within an agreed-upon service area
- Outpatient or home-based therapy experience preferred but not required
