Position Description Non-Exempt
OVERALL OBJECTIVE OF JOB
This position is responsible for case management for indigent individuals in need of services. The Forensic Criminal Case Manager (FCCM) will work directly with indigent individuals who are involved in the criminal justice system. The FCCM will perform a screening/assessment of these individuals to identify areas of need. FCCM will collaborate with community partners, service agencies, and criminal justice professionals to determine available diversionary or reentry services and will formulate reentry/rehabilitation plans for participants to assist with successful reintegration into the community as an alternative to detention as a means of diverting participants from the criminal justice system and assisting in reducing incarceration and recidivism.
Other responsibilities will include writing participation reports as well as creating and maintaining up-to-date individual client files. The FCCM will also collect and maintain statistical data to measure the effectiveness of programs and activities and be responsible for preparing reports detailing this data.
ESSENTIAL DUTIES OF JOB
- Identify clients who will benefit from comprehensive re-entry planning and connections to service providers.
- Conducts participant interviews and administers screening assessment tools to identify the risks and needs of clients.
- Identifies areas of need for individuals which may include mental health, drug and alcohol, housing, and other community supports, and assist individuals in obtaining assessments related to their specific needs.
- Strategizes case coordination with the Public Defender’s office and/or court-appointed counsel.
- Collect and review relevant client documentation/records.
- Develop and assist in implementing case-management plans for program participants.
- Identify challenges to successful re-entry and identify strategies to improve services and outcomes for individuals.
- Create pre and post-release plans for incarcerated individuals to assist with successful reintegration into the community and help reduce incarceration and recidivism.
- Provides information and recommendations to clients regarding available services.
- Engage effectively with program participants and work collaboratively with service providers/criminal justice professionals through both verbal and written communication.
- Collaborate and coordinate with jail staff and correctional professionals to ensure client needs are addressed.
- Establish and develop relationships with community services and potential supports, including the local mental health authority, drug and alcohol service providers, law enforcement, community supervision, the housing authority, adult education services, and community leaders.
- Provides progress reports, both verbally and in writing, when requested.
- Acts as an advocate for clients in interaction with community agencies and organizations.
- Testify in court, when necessary, regarding participants’ needs and progress. Maintains client records and enters data into computer files. Maintain statistics on clients served and recidivism rates.
- Collects and analyzes statistical data for program evaluation to measure effectiveness of programs and activities, as well as preparation of reports detailing this information.
- Travels as necessary to meet with clients, County agencies, and service providers.
OTHER DUTIES OF JOB
- Attend meetings, training, and staffing as required.
- Participates in cross training with staff from County human services agencies, including core training and ongoing continuing education.
- Assists in developing operational materials for forensic case management and associated programs.
- Maintains statistics on work performed and assists in data collection and analysis, as well as preparation of reports to measure effectiveness of programs and activities.
- Performs other job-related duties as required.

WORKING CONDITIONS:
- Works indoors in adequate workspace, lighting, temperatures, and ventilation.
- Works with average indoor exposure to noise, stress, and disruptions.
- Normal indoor exposure to dust /dirt.
- Works in conditions of potential outbursts or disruptive behavior of clients.
- Travels frequently during all seasons and is exposed to outdoor elements, including snow and icy roadways.
PHYSICAL AND MENTAL REQUIREMENTS
- Must possess the ability to record, convey, and present information, explain procedures, and follow instructions.
- Must be able to work in a jail and/or corrections setting.
- Must be able to sit for long periods throughout the workday, with intermittent periods of standing, walking, bending, twisting, reaching, and driving as necessary to carry out essential job duties.
- Dexterity requirements range from simple to coordinated movements of fingers / hands, feet / legs, torso as necessary to carry out essential job duties.
- Sedentary work with occasional lifting / carrying objects with a maximum weight of fifty pounds.
- Must demonstrate emotional stability.
- Must be able to cope with the physical and mental stress of the position.
- Must be able to physically and mentally react quickly in the event of a disturbance or physical outbreak.
- Must be able to pay close attention to details and concentrate on work.

- Must possess the ability to plan and organize work and prepare adequate records and reports.
- Must possess the ability to understand and interpret laws and regulations related to human services programs.
- Must possess the ability to practice good time management and stress management skills and to practice the use of good judgment in assessing needs and serving clients.
