Key Responsibilities:
- The RN MDS Coordinator plays a vital role in ensuring accurate resident assessments and effective optimization of care plans. This position will:
- Lead and coordinate the development and completion of resident assessments (MDS) in line with current regulations and guidelines.
- Ensure the MDS process is completed accurately and within required timeframes.
- Collaborate with the interdisciplinary team to develop personalized care plans for residents.
- Continuously monitor and document residents' progress and responses to care.
- Actively engage in the Quality Assurance and Performance Improvement (QAPI) process to enhance the overall quality of care.
- Maintain clear and effective communication with residents, families, and healthcare providers to ensure seamless care coordination.
- Stay up-to-date on MDS regulations and guidelines to ensure compliance with industry standards.
This role is critical in maintaining high standards of care and regulatory compliance within the facility.

Qualifications:
- Current RN (Registered Nurse) license required
- RAC-CT (Resident Assessment Coordinator-Certified) certification required
- Strong understanding of MDS 3.0, RAI process, and care planning

- Ability to work independently and collaboratively with a team
