This role focuses on conducting grief assessments, developing individualized care plans, and offering both individual and group bereavement support. Hours: Full-time, Monday-Friday
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
- Assessment and care planning: Conduct comprehensive assessments to understand a patient's emotional, social, and practical needs, then develop personalized care plans.
- Grief support: Provide one-on-one and group support sessions to help patients and their loved ones navigate grief and loss.
- Support group facilitation: Plan, coordinate, and facilitate support groups for those experiencing similar losses.
- Community support and outreach: Connect patients and their families with additional resources, perform community outreach, and provide educational programs on grief and bereavement.
- Coordination: Collaborate with other healthcare professionals, such as doctors, chaplain, and social workers, to ensure holistic care.
QUALIFICATIONS:
Education and/or Experience
- Degree in a related field such as social work, psychology, or counseling is preferred.
- Experience in hospice care, grief support, and working with terminally ill patients and their families.
Other Competencies
- Strong communication and people skills, empathy, and compassion are essential.
- Familiarity with local community resources is important for referring clients to additional support services.
- Experience with Assessment and Case Management preferred.
- Experience with Group Facilitation and training preferred.
- Participates in professional and community organizations representing Franciscan Healthcare in a positive manner.
