Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or wheelchairs.
- Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report any abnormalities promptly.
- Support medication administration following prescribed instructions while maintaining accurate medical documentation in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, or memory impairments through tailored activities and attentive supervision.
- Assist with household tasks including cleaning, laundry, meal preparation, and light housekeeping to promote a safe and hygienic environment.
- Document all patient interactions and care activities in compliance with HIPAA regulations to ensure confidentiality and proper medical records management.
- Collaborate with nursing staff and other healthcare professionals to coordinate comprehensive home health services and ensure seamless patient care delivery.
Qualifications
- Proven experience in home health care, assisted living facilities, nursing homes, or long-term care environments.
- Strong knowledge of medical terminology and caregiving best practices for patients with disabilities or chronic illnesses.
