What are the responsibilities and job description for the Case Manager - PMIC position at Four Oaks Family & Children Services?
What you will do?
As the Case Manager you will be responsible for the planning and treatment of individual children, a family, or assigned groups by performing a variety of regular tasks. Specific responsibilities include:
- Developing a care plan for each child containing goals and objectives with projected dates of accomplishment
- Involving the clients, referral agency and family in planning whenever possible
- Developing a specific plan relating to the involvement of the child’s parents.
- Participating in the documentation of case assessment.
- Documenting the implementation of the care plan.
- Providing for scheduled in-person conferences with each resident.
- Providing a supportive atmosphere for the youth with leadership and guidance to the client.
- Coordinating internal and external activities of the youth.
- Serving as a liaison with the referring agency.
- Providing a mechanism for dealing with day-to-day program operations.
- Oversees and maintains general health and well-being of the child
- Overseeing the living activities of the child.
- Monitoring and recording behaviors on a daily basis.

What you need:
To qualify you need a Bachelor's Degree in social work or a degree in a human services related field plus two years of experience in child welfare or a Master's Degree in Human Services is required. Additional requirements include:
- Able to be self driven/reliant and work independently as well as part of a team
- Strong organizational skills
- Ability to show empathy/compassion to build trust based relationships
- Effective verbal and written communication skills
