What are the responsibilities and job description for the Patient Navigator (SUD Specialist) position at Four County Mental Health Center?
Patient Navigator (SUD Specialist) – This position will provide care coordination to individuals and families experiencing problems related to substance use disorders. This position is responsible for collecting, managing, and reporting required data related to CCBHC (Certified Community Behavioral Health Clinic) required outcomes and measures. This requires both interpersonal and data management skills. The patient navigator assures that patients are connected with the right programs and services and seeks to close referral gaps that patients may encounter while receiving care in the CCBHC clinic. The patient navigator must be able to work collaboratively with clients, families, internal programs, external providers, and all aspects of the CCBHC network. The patient navigator must be an outcome focused individual that seeks to help clients have an effective and efficient experience while receiving care. This position is geared towards increasing access to and improving the quality of community mental health and substance use disorder services in pursuit of healthier communities.
A Bachelor’s degree or equivalent combination of education and/or experience in the human services field is required.
Hourly position: starting wage is $50,000.00/year – $24.04/hour.
