What are the responsibilities and job description for the Physical Therapist position at Foundations Physical Therapy?
We are seeking a Physical Therapist to provide outpatient care for orthopedic patients in a mixed evaluation and treatment caseload. Reporting to the Clinic Director, this role will deliver evidence-informed therapy, support patient progress, and contribute to coordinated care while benefiting from mentorship, training, and continuing education opportunities.
Responsibilities
- Evaluate patients and develop individualized treatment plans.
- Provide therapeutic exercise, manual therapy, and neuromuscular reeducation.
- Monitor patient progress and adjust interventions as appropriate.
- Educate patients on treatment plans, exercises, and self-management strategies.
- Complete accurate clinical documentation in electronic health records.
- Coordinate care with patients and other care team members.
- Support effective discharge planning.
Preferred Qualifications
- 1 years of experience in physical therapy.
- State Physical Therapist License.
- Experience with physical therapy assessment, therapeutic exercise, and manual therapy.
- Knowledge of neuromuscular reeducation and clinical reasoning.
- Experience with electronic health records and clinical documentation.
- Strong patient education and communication skills.
