Job Summary
We are seeking a compassionate and dedicated Hospice Social Worker - PRN (as needed) to join our team and provide vital psychosocial support to patients and their families navigating end-of-life care. Location: New Iberia and surrounding Acadiana parishes. In this role, you will assess emotional, social, and behavioral needs, develop personalized care plans, and coordinate resources to enhance quality of life. The ideal candidate will bring a strong background in social work, hospice care, and mental health counseling, with a passion for delivering empathetic patient-centered support.
Duties
- Conduct comprehensive patient assessments to identify psychosocial needs, including mental health status, family dynamics, and community resources.
- Provide individual, family, and group counseling sessions utilizing techniques such as motivational interviewing, cognitive behavioral therapy, and behavioral therapy.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans that address emotional, social, and spiritual needs.
- Assist patients and families with crisis intervention, grief support, and coping strategies during difficult times.
- Manage medical documentation accurately within electronic medical record (EMR) systems while ensuring compliance with HIPAA regulations.
- Coordinate community resources for home care, assisted living, or inpatient services as needed.
- Support clinical research efforts related to hospice care practices or behavioral health interventions when applicable.
Qualifications
- Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited institution; licensure or certification preferred.
- Proven experience working in hospice care, home & community care settings, or inpatient facilities with a focus on end-of-life support.
- Strong knowledge of medical terminology, medical records management, ICD coding (International Classification of Diseases), ICD-9/10 codes.
- Demonstrated expertise in child & family counseling, mental health counseling, crisis management, and behavioral health interventions.
- Familiarity with EMR systems and data collection methods for clinical research or quality improvement initiatives.
- Excellent communication skills with the ability to work effectively with diverse populations including seniors, individuals with disabilities or developmental disabilities, autism spectrum disorder (ASD), Alzheimer’s disease/dementia care, PTSD care, or post-acute care patients.
