Case Manager, Incentive Programs 100% Remote within California, Must reside in California We are seeking a Case Manager to join FoundationCCC in its mission of benefiting, supporting, and enhancing the California Community Colleges—the largest and most diverse system of higher education in the nation. What You’ll Do The Case Manager is responsible for providing comprehensive case management services to participants of various air quality programs, such as Replace Your Ride or Driving Clean Assistance Program, which offer incentives to eligible consumers who wish to replace their older, high polluting vehicle with a newer, more emission-friendly vehicle. This position performs in depth assessments to determine eligibility and qualification, provides oversight of the entire case, links applicants to appropriate services, such as participating in car dealerships and dismantlers, follows up with applicants and partners as necessary, and documents case outcomes using various software applications. The work done in this role will help ensure full compliance with program guidelines, policies and procedures. Provide holistic and comprehensive case management services to eligible consumers including; initial assessment, referral to services, outcome evaluation and continuous follow-up via phone and email as necessary Evaluate documentation related to vehicles sales and vehicle dismantling for accuracy, completeness and compliance with program guidelines and procedures Establish and maintain relationships with contractors and vendors, and performs quality control checks associated with program operations and claims processing Attributes for Success Experience in case management and/or evaluation, analysis and procedural compliance Strong communication, prioritization, customer service, time management, multitasking, risk-management and outstanding analytical skills required Knowledge and experience working with Salesforce or similar CRM systems.
