Job Title:       Registered Nurse (Part-time)

Department: Health Care

Reports To:   Director of Nursing

FLSA Status: Non-Exempt (Hourly Wages)

 

Basic Purpose:  In keeping with Woodlands’ tradition, we seek to enhance the quality of life and holistic growth of seniors by providing high quality service in a cheerful, timely manner and by communicating, coordinating, and cooperating with others to attain goals.  A Registered Nurse provides nursing care in an environment which promotes their rights, dignity, and freedom of choice

Job Summary:  A Registered Nurse provides quality nursing care to residents; implement specific procedures and programs; coordinate work within the department, as well as with other departments; report pertinent information to the immediate supervisor; respond to inquiries or requests for information; assist the immediate supervisor with tasks to support department operations.

Performs other appropriate related and unrelated assigned duties.

Education:  High School Diploma or equivalent; successful completion of a West Virginia state-approved Registered Nursing program

Licensure/Certification:  a current Registered Nursing license in West Virginia; current CPR certification preferred

Experience:  Six months' prior RN experience in long-term care preferred

Qualifications:

  • Cooperates and works together with all co-workers; plan and complete job duties with minimal supervisory direction, including appropriate judgment
  • Maintain confidentiality of all resident care information
  • Interact effectively with residents, guests, supervisors and peers
  • Must be able to cope with mental and emotional stress related to working with persons who are ill, disabled, elderly, and/or emotionally upset
  • Is knowledgeable of resident rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all residents in a safe, secure environment
  • Follow safety regulations, reporting injuries or unsafe conditions and work practices to supervisor
  • Follows up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems and concerns
  • Follow acceptable standards of personal hygiene and comply with department dress code
  • Possess a fundamental understanding of sanitation and cleanliness
  • Regular, punctual and dependable attendance
  • Provide assistance to and work directly with staff, residents, family of residents, visitors, government agencies and the general public

 

DUTIES AND RESPONSIBILITIES

Leadership:

  • Support and promote the company’s Mission Statement
  • Report to the DON all resident care activities, personnel issues, staffing patterns and other relevant topics
  • Encourage staff autonomy by building trust and support
  • Complete additional tasks as assigned by the DON
  • Report to Medical Director in absence of the DON

 

Staff Development:

  • Coordinate the orientation of all staff, document the orienting process. Ensure all training is in compliance with regulatory agency requirements
  • Conduct monthly and additional inservice for all staff when assigned by the DON
  • Train all staff on techniques developed to maintain resident independence and higher functioning
  • Through observation evaluate staff skills and implement corrective actions when necessary
  • Promote and elicit quality improvement suggestions from staff and advance to the DON
  • Conduct and sign work physicals for new hires
  • Audit sign in and out sheets on the floor
  • Monitor yearly TB screen test for staff

Resident Care:

  • Maintain the comfort, privacy, and dignity of each resident in the delivery of services. Interact with residents in a manner that displays warmth and promotes a caring environment
  • Assist in maintaining a safe, neat, and clean environment; report environmental deficiencies to the DON
  • Fully understand all aspects of residents rights
  • Study and analyze techniques in resident care, which will motivate and move residents to greater independence as well as higher level of functioning
  • Conduct an assessment on all assigned new residents on the date of admission. Coordinate a specific care plan for each resident within 24 hours of admission
  • Assess resident’s functional status, social, psychological and spiritual needs on an ongoing basis, but especially at move-in and after hospital stay
  • Monitor and update care plans for assigned residents
  • Conduct an assessment on assigned residents monthly according to the regulations. Notify DON, family/MPOA of change in level of care
  • Chart a monthly nursing summary on assigned residents including nursing activities, change in condition, medication changes, and other pertinent information. Train and monitor all designated staff relative to charting guidelines. Maintain in resident files all documentation pertaining to the progress of these issues.
  • Politely and promptly keep MPOA/Residents aware of plan of care and new orders or changes in conditions.
  • Communicate with the DON, physician, and family members in a polite and respectful manner.
  • Monitor the medication ordering and delivery system daily to assure safety and integrity of the system.
  • Administration of medication to the residents according to medication administration protocol. Assure the safety and integrity of the medication system with respect to the guidelines and regulations
  • Complete narcotic counts on assigned residents when needed with the LPN. Monitor availability of Controlled Substance and inform DON when more are needed from the Safe
  • Conduct daily wound rounds and document wounds and treatment as assigned by the DON
  • Construct weekly RN note on assigned residents with waiver
  • Take new verbal orders from Physicians and Providers and document in all appropriate places
  • Assist the DON in monthly change overs on assigned residents
  • Cover the floor for nursing duties when assigned by the DON
  • Assist with the transfer, discharge, and admission of residents according to OHFLAC & WRC guidelines.

 

Compliance:

  • Follow established safety regulations and use equipment and supplies in a safe manner
  • Maintain confidentiality of all resident or employee information as required
  • Audit MARs daily for assigned residents for holes
  • Audit MARS, new physician orders, and nurse charting practices monthly to assure compliance with regulations
  • Complete all incident reports on a timely basis and forward a copy to the DON daily, as well as to the appropriate state regulatory agency within the specific timeline
  • Fill out complaint/grievance forms
  • Report all complaints/grievances to the DON, the Medical Director and Resident Social Worker

 

Weekend Coverage:

  • Provide rotating weekend RN coverage as assigned by the DON
  • Manage staffing issues to assure adequate nursing staff coverage
  • Notify the DON of the following incidents that need further actions:
  1. Call system down
  2. Power outage
  3. Complaints
  4. Significant med error that caused harm to residents
  5. Significant incident that caused harm to residents, like a fall resulting in fracture
  6. Narcotic diversion
  7. Anything that affected or may affect resident safety

 

PHYSICAL DEMANDS

  • Requires extensive physical effort; standing, grasping, balancing, carrying, bending, lifting, stretching, stooping, pulling and pushing
      • Trays 10 lbs.
      • Tools, Equipment, Supplies 20 lbs.
      • Transfer of Residents, ambulation of residents, and equipment 50 lbs.
  • Requires lifting of residents (sometimes alone and sometimes with a partner)
  • Standing frequently during the 8 hour shift
  • Frequent walking, stooping, bending
  • Occasional reaching above the head
  • Level II exposure to blood borne pathogens

WORKING ENVIRONMENT

  • Level II Exposure to blood borne pathogens
  • Work area is generally well lit and ventilated
  • Work location as assigned (in Woodlands’ Health Care Wing)
  • May be required to work additional hours as dictated by the workload and staffing; must be willing to work evenings, weekends and holidays
