Job Title: Social Care Network (SCN) Performance Liaison
Reports to: SCN Performance Coordinator
Project Summary: The New York State Department of Health has established Social Care Networks (SCNs) as part of the 1115 Waiver Demonstration Amendment, and Forward Leading IPA (FLIPA) is the SCN Lead for the Finger Lakes Region. This region is comprised of 14 counties, including Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming, and Yates. The SCN’s aim is to enhance the delivery of social care services to Medicaid members by coordinating efforts among community-based organizations (CBOs) and other health care partners to create a more resilient, flexible, and accessible social care system that reduces health disparities and advances health equity. The SCN will collaborate with CBOs and other health care partners, leveraging shared data and technology to coordinate social care services for Medicaid members to improve access, ensure reliable and timely referrals, streamline and track navigation and completed referrals in closed loop systems and enhance collaboration between social care service providers and other regional partners.
Job Summary: The Performance Liaison will support the Performance Coordinator in ensuring the FLIPA SCN (WeLinkCare) and engaged CBOs meet performance and compliance milestones through an auditing and monitoring process. This position is grant-funded through March 2027.

Duties/ Responsibilities:
- Work with the Performance Coordinator and Data Team to support preparation of dashboards to assess and monitor network level performance and CBO performance
- Leverage identified data collection platform(s) to assess for timely and accurate submission of Medicaid Members’ needs and information (e.g. perform routine audits of Medicaid Member profiles in referral system)
- Monitor progress of onboarded CBOs and the WeLinkCare Navigation Hub
- Maintain tracking of required onboarding and training activities (e.g. pre-onboarding requirements, implementation status, and completion of required training) and monitor that new system users complete required training activities
- Provide clear ongoing communication with the Performance Coordinator and other key SCN staff on status of onboarding, implementation, and audit results
- Serve as one point of contact for external entities needing information to support Medicaid Member care management and SCN status
- Help identify challenges and opportunities for improvement related to training and support identification of technical support needs based on results of progress monitoring
- Support preparation of reporting summaries for communication in a variety of forums as needed and support preparation of CBO performance reviews
- Effectively work across multiple organizations and geographies to support project progress within different organizational culture structures and communities
- Other duties as assigned
Education & Experience:
- Bachelors in related field, such as human services, social work, public health or similar, or equivalent experience required
- 3-5 years of community collaboration, interdisciplinary coordination or related experience required
- Experience within the New York State health care system (population health, behavioral health, Federally Qualified Health Centers, or similar) required
- Prior experience managing performance within large-scale grants/projects preferred
Skills, Knowledge, and Abilities:
- Ability to manage multiple priorities effectively with minimal supervision
- Outstanding communication and people skills that reflect person-centered best practices and individual autonomy
- Demonstrate an understanding of underserved populations and their needs
- Knowledge and experience with reporting packages and databases
- Strong analytical skills with the ability to collect, organize, analyze and disseminate information in both written and oral communication
- Strong interpersonal skills, independently resourceful and thrives in a fully remote environment
- Proven ability to complete projects according to defined timelines and objectives
- Proficient with MS Office and various business software
Salary Range: Salary is commensurate to education and experience with a range of $55,000- $65,000.
Additional information: This position will be remote with regular travel expected for on-site meetings or training across the Finger Lakes Region and New York State
