Salary Range: $27.48 to $30.22 per hour (approximately $50,000 - $55,000 annually)

Location: Long Island City, Queens 

Days/Hours: Monday through Friday 9:00am to 5:00pm, with 1 remote/field day (after 90 days of employment)

Essential Duties and Responsibilities:

Position Summary: 

GROWTH POSITION! The Fortune Society is seeking an innovative, client-centered, strategic thinker to join our Behavioral Health team. Individuals in this in-person position will have overall day-to-day responsibility for connecting justice-impacted individuals, many who have complex health conditions, to a wide range of medical, mental, and social determinants of health services. This is a growth position for the right candidate who demonstrates a strong work ethic and a willingness to think outside of the box for the betterment of his/her assigned caseload.  

** MASTERS GRADS: This is an excellent opportunity for new MSW graduates looking to enter the social services field and potentially move into Fortune’s outpatient Treatment Service clinic or Mental Health Outpatient Treatment and Rehabilitative Services (MHOTRS) programs once licensure is received. 

Essential Duties and Responsibilities: 

• Coordinate Plan of Care for clients with criminal justice histories and chronic medical and/or mental health conditions; 

• Screen all participants assigned for eligibility into Fortune’s Care Management Unit (CMU); 

• Offer supportive services such as, peer services, medication assistance/linkages to healthcare housing screenings and assistance with the HRA 2010e Supportive Housing Application;  

• Work closely with Fortune’s interdisciplinary care team including the PCP, psychiatrist, therapist, residential service providers, DHS case workers, substance abuse treatment programs, ACT teams, Social Care Network (SCN) partners, care team, and/or family members, etc., to determine appropriate service referrals; 

• Under supervision of the director, ensure that all services delivered are within the scope of the Plan of Care and that accurate records of needs/progress/regress are maintained, as per NYS DOH Medicaid guidelines; 

• Include input from providers/client/client’s family for written individualized Plans of Care and consult with family members, parole and probation as necessary; 

• In conjunction with the client and Behavioral Health and Care Management team, identify potential barriers to care and strategies for overcoming those barriers, including those relating to medication compliance; 

• Regularly liaise with Fortune’s housing screening team to identify potential placement opportunities for eligible individuals; 

• Complete all requisite agency and Health Home care management trainings; 

• Perform additional responsibilities as needed. 

Qualifications: 

• Minimum of a bachelor's degree is required, or High School diploma or GED with at least 2 years of experience in healthcare, social work, or mental health field; college graduates with strong internship or field experience will also be considered; 

• Ability to demonstrate strong written and verbal communication skills through reports, correspondence and presentations; 

• Strong organizational and interpersonal skills; ability to manage competing priorities within a growing unit; 

• Proficiency in MS Office, SharePoint, and Excel (working knowledge of MAPP, ePaces, PSYCHES and Foot Hold plus, SPARS); 

• Bilingual English/Spanish strongly preferred to best suit the clients who reside in this area; 

• Ability to perform additional responsibilities as needed;  
    

Travel Requirements Will require travel throughout the five boroughs, as well as to Fortunes locations in Harlem and Long Island City. May require additional travel to clients’ locations, including those within the shelter system, when necessary, to ensure continued progress of Plan of Care. MetroCard transportation is provided to staff for all client-related activities.
