About the Role
We are seeking a skilled and compassionate Speech Language Pathologist (SLP) to provide high-quality therapy services to patients in the home setting. In this role, you will evaluate patients, develop and implement individualized treatment plans, monitor progress, and educate patients and caregivers while ensuring compliance with physician orders and supporting safe discharge planning.

Responsibilities

Provide skilled speech therapy services in the home setting in accordance with the physician’s plan of care.

Evaluate, treat, and regularly re-evaluate patients’ speech, language, cognitive, and swallowing needs.

Develop, implement, and update individualized plans of care based on patient progress and clinical assessment.

Document patient responses to treatment and communicate changes in condition to the physician and clinical leadership.

Educate patients, families, and caregivers on therapy goals and strategies to support care.

Collaborate with the interdisciplinary team and make referrals to other disciplines as appropriate.

Ensure services are delivered safely, effectively, and in compliance with agency policies and standards.

Physical Requirements

Ability to travel to patient homes and work in varied home environments.

Must be able to stand, sit, bend, and lift up to 15 pounds as needed.

Requires clear communication skills and use of a tablet (company provided) for documentation.

Qualifications:Qualifications
- Graduate of an accredited college with a Master’s or Doctoral degree majoring in speech-language pathology and is currently licensed in Utah (required)
- Holds a certificate of clinical competence and licensure granted by the American Speech and Hearing Association (required).
- Minimum of two years speech pathology experience (required)
