About the Role
We are seeking a compassionate and reliable Hospice Licensed Clinical Social Worker (LCSW) to support patients and families served by Fortis Health–affiliated agencies. In this role, you will provide professional medical social services in the home, addressing social, emotional, and psychosocial needs while supporting patient dignity, comfort, and quality of life in accordance with the plan of care.
Responsibilities

Provide medical social services to hospice patients and families in the home setting.

Assess and address social, emotional, and psychosocial needs related to illness and end-of-life care.

Collaborate with the interdisciplinary team to develop and support the patient’s plan of care.

Counsel patients and families to support coping, adjustment, and decision-making.

Communicate changes in patient condition to the supervisor and physician as needed.

Connect patients and families with appropriate community and social service resources.

Participate in care coordination, discharge planning, and team meetings.

Supervise Bachelor’s-level social workers, as applicable.

Qualifications:

Master’s or Doctoral Degree in Social Work from a CSWE-accredited program (required).

Current Licensed Clinical Social Worker license in the state of practice (required).

Active NPI (required).

At least (2) years of healthcare social work experience, hospice or home health (preferred).

Strong communication, organization, and interpersonal skills.
