About the Role
We are seeking a compassionate and reliable Licensed Clinical Social Worker (LCSW) to support patients and families served by Fortis Health–affiliated home health agencies. In this role, you will provide professional medical social services in the home, addressing social, emotional, and psychosocial needs related to illness, recovery, and care planning. You will support patient dignity, independence, and quality of life while working in collaboration with the interdisciplinary team and in accordance with the established plan of care.
Responsibilities

Provide professional medical social services to patients and families in the home environment

Assess and address social, emotional, and psychosocial factors impacting health status

Counsel patients and families to support coping, adjustment, and treatment adherence

Collaborate with the interdisciplinary team to develop and re-evaluate the plan of care

Report changes in patient condition and response to treatment to the supervisor and physician

Participate in care coordination and discharge planning

Utilize appropriate community and social service resources to support patient needs

Serve as a resource to team members in identifying and resolving patient concerns

Participate in staff meetings, in-service programs, utilization reviews, and staff development activities

Maintain required continuing education in compliance with state and federal regulations

Supervise Bachelor’s-level social workers, as applicable

Qualifications:-
Master’s or Doctoral degree in Social Work from a CSWE-accredited program (required).

Active LCSW license in the state of practice (required).

Minimum of (2) years of social work experience in a healthcare setting (required).
