About the Role
We are seeking a compassionate and reliable Hospice Certified Nursing Assistant (CNA) to support patients served by Select Hospice. In this role, you will provide hands-on personal care in the home, focusing on comfort, dignity, and quality of life while assisting patients and families during end-of-life care under the direction of the RN.
Responsibilities

Provide personal care including bathing, dressing, oral care, feeding, and elimination

Assist with ambulation, positioning, transfers, and ADLs

Measure and document vital signs and observations as directed

Travel to and care for multiple hospice patients per day in their residences, following individualized plans of care

Observe and report changes in patient condition or home safety concerns

Follow RN care plans and agency policies

Maintain patient confidentiality and HIPAA compliance

Complete required trainings, meetings, and in-services

Support patient comfort through socialization and emotional presence

Qualifications:

High school diploma or equivalent (preferred).

Current CNA license in the state of Indiana (required)

Minimum 1 year of direct patient care experience (required)

Listed on the Nurse Aide Registry with no findings of abuse or neglect
