Fort Tryon Center is a Skilled Nursing Facility in Washington Heights.
We seek an energetic and professional social worker and/or a case manager to join our team.
This position consists of working with our residents to enhance their psychosocial well-being and ensuring that they are receiving the highest level of care.
The position includes assisting residents to return to the community after their rehabilitation has been completed.
The position is fast paced so the individual must be organized & be able to multi task. Good communication skills are required, both orally and in writing.
Experience with Sigma Care or other electronic medical records are a plus.
Bi-lingual (Spanish) is preferred.
New graduates are welcome.
Salary is commensurate with experience with benefits negotiable.
