DATA MANAGEMENT COORDINATOR (DIABETES PROGRAM)

Hourly Range: $26.13/hr - $32.14/hr

ESSENTIAL DUTIES, FUNCTIONS AND RESPONSIBILITIES

• Maintains existing programs and updates the data collection processes that will contribute to meeting the goals and objectives of the Diabetes Program. Employee collects and manages data so that it can be used to analyze business functions, productivity and redundancies. Works to ensure that the data is collected properly, following HIPAA standards.
• Creates and maintains the Diabetes Registry using RPMS, including creating the search logic using ICD-10 and CPT-4 codes related to diabetes as well as other data entry systems.
• Consults with the Diabetes Program Manager, Diabetes Program staff and other medical, professional and technical staff by developing and applying methods through the collection and storing of data in a database through the use of staff/work computers and various programming (QMan, DMS, iCARE, RPMS, VGEN, BPRM, etc.) options involved in such use.
• Attends meetings and represents department or program, as requested.
• Provides professional data presentation when requested by Diabetes Program Manager during staffing meetings, or local and community level events.
• Prepares documentation of the database systems and instructs system users and staff in their operation. Creates and maintains documentation of the detail logic and operation of programs developed.
• Uses the Diabetes Management System (DMS) of registry maintenance, flow sheet setup, add patient from template, add/edit DMS letters, entering/deleting patients from the registry, patient management, report generation, diabetes program audit, and retrieval of clinical data from the PCC for patients in the registry.
• Utilizes the Diabetes Management System User Manuel to extract data related to: Diabetes Standards Health Summary, Pre-Diabetes Patient Care Summary, Individual Audit, Cumulative Audit, Special Diabetes Program for Indians (SDPI) Key Measures Report, Updating patients in Registry, Taxonomy Setup, Queries and Reports, creating labels for templates of RPMS Diabetes Patients, Purpose of Visit codes and understanding DMS PCC Data Entry Mnemonics.
• Utilizes QMan User Manual and will understand the annual release notes, demographic attributes and pre-diabetes and diabetes audit logic.
• Extracts data from QMan related to diabetes and gestational diabetes such as; measurements involving tests, and screens, diagnosis and problem list entries, prescriptions and patient education, immunizations, dental and medical procedure, visits and providers, hospitalization and Patient Referred Care (PRC) services, contraception methods and pregnancy history, and contract information.
• Provides support, guidance, and assistance for grant funded departments with data management and data extraction under the direction of the Diabetes Program Manager.
• Ensures accuracy of impromptu details and ensure that what is being recorded and stored is correct.
• Uses and navigates the Electronic Health Record (EHR) to view pertinent patient information to update and maintain the DMS registry.
• Generate data reports for diabetes education accreditation program (DEAP), grants, Quality Assurance/Performance Improvement (QAPI) projects, GPRA, and any additional initiatives.
• Utilizes the RPMS system as well as QMan and PCC Reporting to ensure that required data is valid and extractable.
• Ensures compliance with Privacy and HIPAA standards and supports compliance with established CMS Standards.
• Other duties as assigned.
  

  
  MANDATORY MINIMUM QUALIFICATIONS

  Experience: 

  Two (2) years of direct work experience in data management within the healthcare field.

  Education:

  Bachelor’s degree in Computer Information Technology or related field, or equivalent experience.
