Salary Range: $116,732.00 - $151,751.00/per annum
Experience: Three (3) years of direct work experience in quality improvement in a health care setting, data analytics or care coordination in a Primary Care setting.
Education: Bachelor’s Degree in Healthcare Administration, Nursing, Public Health, or closely related field.
PMCH certification (Joint Commission, AAAHC, or NCQA).

- APPLICANT MUST HAVE A VALID, UNRESTRICTED INSURABLE DRIVERS LICENSE**
- RESUMES AND REFERENCES ARE REQUIRED**
- Develops, strategizes, implements, and maintains program initiatives, assessments for evaluation and organizational standards of satisfaction, quality, and performance to meet PCMH practices.
- Ensures security and privacy of data in all FDIHB, Inc. associated systems.
- Lead PCMH transformation and ongoing compliance with accreditation standards.
- Collect and submit data to AHCCCS for American Indian Medical Home (AIMH) designation.
- Prepare and manage documentation for PCMH recognition/accreditation.
- Monitor adherence to policies, workflows, and care delivery standards.
- Support care coordination across providers, specialists, and community resources.
- Promote patient-centered care, including shared decision-making and self-management support.
- Track, analyze, and report clinical quality metrics.
- Lead quality improvement initiatives.
- Identify gaps in care and assist with corrective action plans.
- Facilitate interdisciplinary team meetings with stakeholders.
- Train staff on PCMH principles and workflows.
- Serve as a liaison between leadership, providers, nurses, and community resources.
- Utilize the Electronic Health Record (EHR) system to extract and analyze data.
- Ensure accurate documentation and data integrity.
- Performs other duties as assigned.
