Essential Duties, Functions And Responsibilities

• Serves as a technical resource to Navajo Chapters, Schools, and Communities to revitalize community gardening projects.
• Works with local farmers, vendors, Navajo Chapters, schools, and communities to organize, plan, and implement Farmer’s Markets within the FDIHB service delivery area.
• Serves as a liaison between health and social services and the community to facilitate access to services and to improve the quality and cultural competence of public health service delivery and programming.
• Provides HPDP program health information and health/wellness/nutrition education to various community organizations and schools.
• Plans, conducts, implements, and participates in community visits/activities, school visits/activities, work site visits/activities, and healthcare facility visits/activities.
• Assists with special health events/activities within the community, schools, Chapters, etc.
• Assists local Navajo Chapters with their Healthy Diné Nation Act (Junk Food Tax Initiative) Health and Wellness Projects.
• Performs outreach for community health education and markets upcoming events and programs at Chapters, schools, and organizations.
• Provides health education sessions to classrooms, schools (student body presentations), Chapters, Senior Centers, Head Start Programs, and other community organizations.
• Maintains and updates a comprehensive inventory of various health educational materials, curriculums, and related multimedia sources.
• Provides assistance to HPDP and Division of Healthy Living and Outreach program staff with various public health activities and interventions.
• Participates in community outreach and planning activities such as survey dissemination to determine community health needs and collects statistical information for grant reporting requirements and needs assessments.
• Assists staff with precise, accurate, and complete data entry, collection, documentation, and reporting.
• Builds and maintains positive working relationships with the Chapters, schools, partners, and related organizations.
• Expands knowledge and understanding of community resources, services and programs.
• Links underserved communities to various health and social service systems of care and helping them navigate these systems to overcome access to care barriers.
• Advocates for services to meet community needs, and empowering individuals and communities to advocate for their healthcare needs.
• Participates in special public health projects and activities within the hospital/clinics, schools, Chapters, and other related organizations.
• Receives formal training and implements/teaches various public health prevention programs (Botvin Life Skills, Diabetes Education, PAK, CATCH, Navajo Wellness Model, Suicide Prevention Methods, and related programs).
• Performs other duties as required.
  
  

Mandatory Minimum Qualifications

Experience: Two (2) years of direct work experience.

Education: High School Diploma or High School Equivalency (HSE).

FDIHB and its facilities are located within the Navajo Nation and, in accordance with Navajo Nation law, has implemented a Navajo/Indian Preference in Employment Policy. Pursuant to this Policy, applicants who meet the minimum qualifications for this position and who are enrolled members of the Navajo Nation will be given primary preference in hiring and employment for this position and members of other federally-recognized Indian tribes will be given secondary preference.
