Job Summary:
We are seeking a skilled professional to provide case coordination and case management services to veterans, spouses/beneficiaries, and dependents. The successful candidate will work closely with clients and families, conducting psychosocial and resource needs assessments, and participating in outreach activities and collaboration meetings.
Responsibilities:
- Conduct thorough client and family psychosocial and resource needs assessments to identify areas of support.
- Develop and implement client service plans, monitoring progress and adjusting as needed.
- Provide brief counseling, crisis intervention, and motivational interviewing to individuals and families to promote self-sufficiency and well-being.
- Foster relationships with community resources to ensure clients have access to necessary services.
- Prioritize case reviews, advocating for clients and collaborating with other service providers.
- Deliver interventions using a client-centered approach, educating and training clients as needed.
- Participate in quality assurance activities to evaluate the effectiveness of service delivery systems.
- Maintain current knowledge of resource availability, costs, and budgetary parameters.
Qualifications:
- Bachelor's degree in Social Work, Counseling, or a related field, or equivalent experience.
- At least 2 years of experience working with vulnerable populations in social services.
- Strong written and verbal communication skills, with ability to work effectively in a team environment.
- Ability to travel into the field and participate in outreach events as needed.
- Knowledge of case management software, grant funded programming, and psycho-social assessments is preferred.
