What are the responsibilities and job description for the Public Health Nurse II (Care Management - HRP) position at Forsyth County (NC)?
The Forsyth County Department of Public Health is seeking to hire a full time Public Health Nurse II. We are looking for a qualified individual to work in our Care Management for High-Risk Pregnancies (CMHRP) program. The primary purpose of CMHRP Public Health Nurse II is to administer care management services for Medicaid recipients deemed as at risk for adverse birth outcomes. Emphasis is placed on improving birth outcomes through reducing the rate of preterm and low birthweight births and monitors the pregnant Medicaid beneficiary population and prenatal service delivery system using data. CMHRP applies systems and information to improve care and assists members in becoming engaged in a collaborative process designed to manage medical, social and behavioral health conditions more effectively. Meeting the diverse and complex needs of members requires a holistic, person-centered approach that addresses both physical, social and behavioral health. A holistic approach must consider the social determinants of health (SDOH), which are "conditions in the environment in which people are born, live, learn, work, play, worship and age that affect a wide range of health, functioning and quality-of life outcomes and risks" (Office of Disease Prevention and Health Promotion, 2016). The more complex the needs, the more comprehensive the approach should be for assisting the member with a care plan that addresses the whole person and collaborates with other systems that impact the member’s wellbeing.
This position utilized county vehicles for patient visits at the patient's home, meeting the patient in the community and going to the OB assigned practice. A successful candidate should have the following knowledge, skills and abilities:
Through knowledge of the principles of care management for a broad spectrum of social medical and financial issues.
Ability to assess the appropriateness of care management bases on established guidelines and projected outcomes.
Ability to communicate effectively with clients, providers and community partners.
Ability to facilitate interdisciplinary collaboration with positive outcomes for the client.
Ability to formulate and execute a plan of care individualized for the client.
Ability to develop and maintain effective working relationships with client, staff and providers.Graduation from a four year college or university with a B.S. Degree in Nursing which includes a public health nursing rotation and one year of professional nursing experience in specialty area; or
Graduation from an accredited school of professional nursing and two years of professional nursing experience in specialty area assigned.
Must be licensed to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing.
A higher education level may be considered as a substitution for all or part of the experience requirement.
A valid Drivers License is required.
Essential duties and responsibilities include but are not limited to the following:
Monitoring and evaluating client progress toward mutually established goals;
Providing coordination of client's care, services and educational efforts emphasizing community collaboration;
Meeting with clients to formulate and execute a plan of care and self-management;
Utilizing targeted reports to identify clients who may benefit from care management;
Screening clients to assign priority risk and entering the data into an approved documentation system;
Assessing whether a client will benefit from care management based on projected outcomes, utilization or cost and risk factors;
Completing an assessment on all clients with a positive risk factor or priority indicator;
Communicating on behalf of the client with PMH, Personal Care Physician, DSS, Health Department, hospital staff, community providers and others providing care to the client and;
Reconciling and monitoring client compliance with prescribed medications(s).
